The TRIUMPH-1 retatrutide trial provides phase 3 clinical evidence about the effects of retatrutide on body weight and selected obesity-related conditions.
Published in the New England Journal of Medicine on 29 September 2026, the study enrolled 2,339 adults and followed participants for 80 weeks.
Its findings extend the earlier phase 2 research, but the trial’s design, endpoints and statistical methods are essential for understanding what the results establish.
What Was the TRIUMPH-1 Retatrutide Trial?
TRIUMPH-1 was a randomised, double-blind, placebo-controlled phase 3 study of retatrutide, also known as LY3437943.
Retatrutide is a synthetic peptide that activates three receptors involved in metabolic regulation: GLP-1, GIP and glucagon receptors.
The trial studied adults with obesity who did not have diabetes. It also included prespecified groups of participants with knee osteoarthritis or obstructive sleep apnoea.
Participants were randomly assigned to retatrutide or placebo for 80 weeks. The study evaluated several treatment groups under a controlled clinical protocol.
The trial is registered as NCT05929066.
What Were the Primary Outcomes?
TRIUMPH-1 examined more than body-weight change.
Its primary outcomes included:
- Percentage change in body weight.
- Change in the WOMAC pain score among participants with knee osteoarthritis.
- Change in the apnoea–hypopnoea index among participants with obstructive sleep apnoea.
The WOMAC score was used to assess osteoarthritis-related pain, while the apnoea–hypopnoea index measured the frequency of breathing disturbances during sleep.
These outcomes were evaluated in their relevant participant groups rather than being treated as interchangeable measures of metabolic health.
The trial’s principal comparisons focused on two of its retatrutide treatment groups against placebo.
What Did TRIUMPH-1 Report About Body Weight?
At 80 weeks, the published study reported the following mean percentage changes in body weight:
| Randomised group | Mean body-weight change |
|---|---|
| Retatrutide 4mg trial group | −17.6% |
| Retatrutide 9mg trial group | −23.7% |
| Retatrutide 12mg trial group | −25.0% |
| Placebo | −3.9% |
These figures come from the published treatment-regimen analysis.
The differences between the 9mg and 12mg groups and placebo were statistically significant.
However, these results describe average changes within the study population. They do not predict how every individual would respond, nor do they establish outcomes beyond the observed study period.
The trial-group designations are included here solely to identify the published experimental comparisons, not to provide administration guidance.
What Did the Study Find About Knee Osteoarthritis?
TRIUMPH-1 included a subgroup of 574 participants with knee osteoarthritis.
Researchers assessed changes in pain using the WOMAC pain score.
The published results showed greater average improvements in the principal retatrutide comparison groups than with placebo.
This finding is relevant because obesity-related clinical research increasingly examines functional outcomes alongside changes in body weight.
Nevertheless, the result does not establish that the same improvement would occur in every person with osteoarthritis.
It also does not independently determine how much of the observed change resulted from weight reduction versus other biological processes.
What Did TRIUMPH-1 Find About Obstructive Sleep Apnoea?
A separate subgroup of 243 participants had obstructive sleep apnoea.
The trial measured changes in the apnoea–hypopnoea index, which records breathing disturbances per hour of sleep.
The published findings indicated greater average reductions in these events in the principal retatrutide groups compared with placebo.
This provides clinical evidence within the population and assessment period studied.
It should not be interpreted as proof of long-term disease resolution or as evidence that every participant experienced the same response.
How Were Safety Outcomes Assessed?
The researchers also recorded adverse events during the study.
Gastrointestinal events were the most commonly reported adverse-event category.
Safety interpretation requires more than identifying the most frequent symptoms. Researchers must also consider event severity, participant withdrawals, observation periods and differences between treatment groups.
An 80-week trial provides substantially more information than a short laboratory experiment, but it cannot rule out uncommon adverse events or establish safety over an indefinite period.

How Does TRIUMPH-1 Differ From Earlier Retatrutide Research?
The 2023 phase 2 obesity trial enrolled 338 participants and reported outcomes over a shorter study period.
TRIUMPH-1 involved a substantially larger population and longer follow-up.
This allows a more detailed examination of clinical outcomes and adverse-event reporting.
However, the two studies used different designs and statistical analyses. Their reported percentages should not be compared without accounting for those differences.
Earlier laboratory research also addressed different questions, including receptor activation and molecular pharmacology.
For example, How Do cAMP Assays Measure Retatrutide Receptor Activation? explains how cellular experiments measure signalling responses.
The article How Is Retatrutide Receptor Potency Measured Using EC50? examines functional potency measurements.
Neither type of laboratory experiment can substitute for clinical trial evidence.
What Are the Limitations of the TRIUMPH-1 Findings?
Several considerations remain important.
The trial evaluated a defined population without diabetes, so its results cannot automatically be generalised to all metabolic conditions.
The knee osteoarthritis and sleep apnoea outcomes came from specified participant subgroups.
The study also used statistical approaches to address events such as treatment discontinuation. Readers should distinguish the treatment-regimen analysis from alternative estimands when interpreting the reported figures.
Finally, the trial was funded by Eli Lilly, the developer of retatrutide. Funding does not invalidate the findings, but disclosure and independent scientific assessment remain important.
The published report supports conclusions about the measured outcomes under the study conditions. It does not establish the long-term safety or clinical suitability of unverified commercial research materials.
Retatrutide Research Products Available in the UK
Peptide Lab Direct UK lists retatrutide laboratory research materials in several nominal strengths:
- Retatrutide 5mg Research Peptide — nominal 5mg research presentation.
- Retatrutide 10mg Research Peptide — nominal 10mg research presentation.
- Retatrutide 20mg Research Peptide — nominal 20mg research presentation.
These are product specification links, not sources of clinical trial evidence. Commercial research products should not be assumed to be equivalent to the characterised materials used in TRIUMPH-1.
The nominal strengths listed above are also unrelated to the experimental group designations in the published clinical study.

What Does TRIUMPH-1 Establish for Retatrutide Research?
TRIUMPH-1 provides peer-reviewed phase 3 evidence of substantial average body-weight reductions and improvements in selected obesity-related outcomes during an 80-week trial.
Its findings strengthen the clinical evidence base for retatrutide while leaving questions about longer-term outcomes, broader populations and uncommon adverse events open to further research.
The study should be interpreted through its original publication, protocol and statistical analyses rather than isolated headline percentages.
